Student Passport System

You will need to upload the pages from your completed Health Form to the Student Passport System.  It will be easiest to have your Health Form totally completed and scanned to a digital copy before following the directions below.  If you have any questions or trouble uploading your documents please contact your program instructor or the Learning Technology Center.

Getting Started:

1. Complete your Health Form in its entirety
2. Scan each section of your Health Form to separate PDFs
3. You will receive an email from Student Passport System to your Northwood Tech student email containing your login information
4. Go to http://studentpassport.clinicalcoordination.org

Once you're logged into your SPS account, go through each section and upload the relevant document(s).

Within each section:

1. Click on "Uploader" to attach the needed PDF(s)
2. Click on "Submit for Review" to complete that section
